THE SATELLITE GUY, INC. Dish Network Offering Terms & Conditions

Overview of possible fees based on actual site survey &/or services requested

MUST BE A NEW 1st TIME RESIDENTIAL CUSTOMER of DISH NETWORK - if you or anyone in your household has had Dish at any time, anywhere, Dish Network will not activate your receivers at time of install and you will be responsible for all activation, equipment costs as well as fees associated with installer services provided to you.
CRAWL SPACE / ATTIC ACCESS FEE - $65

    Standard install covers (1) single wall penetration to get the cable from the dish to the receiver/tv. If it requires more than that, for example, if installer is required to go into the crawl space or attic or go through interior walls, the charge will be $65.00/hr.
LINE RUN/OUTLET FEE - $35

    Ex 1: If existing lines cannot be used, a $35 line run fee for each additional receiver above and beyond the 2 receivers covered under the Standard Installation charge will apply.
    Ex 2: Customers wanting (2) TV’s to share (1) receiver, the fee will be $35 to run the cabling from the receiver to that 2nd TV.
SITE SURVEY FEE - $35

    If for ANY REASON (with the exception of NO SHOW) the Tech arrives at a location and is unable to do the install, there will be a $35 site survey fee assessed. This includes but is not limited to, unable to attain line of sight to the satellite (No Shot), hazardous conditions (snow, ice etc. too thick on roof for ex.), etc.
HANDLING AND DELIVERY FEE- See below

For Hawaii:
    $20 Pearl City, Aiea, Waipahu, Fort Shafter, Kaneohe, Waimanalo, $30 Eva Beach, Kapolei, Makakilo, Nanakuli, Barbers Point, Wahiawa, Mililani, Kunia, Schofield, $50 All North Shore areas north of Kaneohe to Haleiwa & Makaha.
For Alaska:
    $50 Big Lake, $75 Girdwood, $75 if more than 15 miles from Carr’s Wasilla or Carr’s Palmer (round trip). Other fees may apply based on locations outside of these areas..
SERVICE CALL FEE - $65/hour

    Dish equipment, cable & LNB are covered by the Installer for 90 days from date of installation. This warranty does not cover the Receiver. If customer scheduling for a Service Call and the Technician determines the Dish, LNB or Dish cable is not functioning correctly AND it falls within the 90 day warranty period, there will be no charge for the Service Call. If it is determined the malfunction is due to anything other than the Dish, LNB or Dish cable (i.e., the receiver, home wiring, etc.) there will be a $65/hr Service Call fee charged to the customer and payable to the Technician at the time of the Service Call. If the malfunction is with the Dish, LNB or Dish cable BUT it is beyond the 90 day warranty period, the customer will be charged the $65/hr Service Call fee, payable to the Technician at the time of the Service Call.

1. Payment is due at the time of service unless prior arrangements have been made.
2. Should Dish Customer not be at location of install at scheduled time of appointment, THE SATELLITE GUY, INC. will charge the customer’s credit card a $65.00 No Show Fee.
3. THE SATELLITE GUY, INC. is NOT responsible for loss of programming due to malfunctions of satellites. Equipment cannot be returned after 30 days.
4. In a multi-receiver installation, only one receiver is billed the full programming rate. Additional receivers are charged $4.99 each per month to activate for the same programming that is on the primary receiver. All multi-receiver systems must be connected to the same phone number.

 Non-Standard Options: Hook up stereo Additional Receiver or TV hook ups Attic work Home theatre systems Additional cable Second dish installation Trenching Travel over 1/2 hour, Roof mounts Wall fish Conduit High wall mounts Fees and costs for non-standard options are in addition to the standard installation price and are payable to The Satellite Guy, Inc. at the time of service.

Warranty Information: Equipment is covered by the manufacturer’s warranty. The Installer covers the installation for 90 days from date of install. Subject to the exclusions below, there will be no equipment or service charge for equipment replaced or repaired during the warranty period. Installer shall provide warranty and repair services of equipment. This agreement shall become effective upon the installation date of the equipment.

Service Charges: 1. Repairs and maintenance services required beyond the manufacturer’s warranty shall be charged for at Installer’s standard time and material rate plus zone travel charges. 2. All service calls are payable at the time service is completed.

Exclusions: This agreement does not, whether within the warranty period or otherwise, cover the labor, parts or expense necessary to repair damage caused by acts of God, power surges, abuse, misuse, unauthorized modifications, or causes other than ordinary use.

Dish Network Sales Agreement: DISH Network systems are sold at a price that is substantially below our initial cost. In order to purchase a system at THE SATELLITE GUY, INC.’s prices, you must agree to the following conditions of sale. 1. You must be a first time DISH Network residential subscriber 2. You must activate the America’s Top 60 or America’s Top 120 within 7 days of purchase or by the specific ending date of the current promotion, whichever occurs first. You must activate all receivers that you purchase. 3. You must keep all programming and secondary receiver charges (if applicable) current (including receiver fees and PPV) for 12 consecutive months (or 24 Months if you signed up for a  24 month agreement). You must not let this service disconnect FOR ANY REASON. 4. The primary system receiver CANNOT be used as a second, slave or replacement receiver. 5. The highest model number receiver MUST be activated as the main primary receiver. Failure to meet any of these terms and conditions will result in an additional $300.00 charge to you or your credit card and must be paid in full within 10 days of billing. All secondary receivers are charged $60/receiver in addition. THE SATELLITE GUY, INC. Standard Conditions of Sale 1. Payment is due at the time of service unless prior arrangements have been made. 2. Should Dish Customer not be at location of install at scheduled time of appointment, THE SATELLITE GUY, INC. will charge the customer’s credit card a $65.00 No Show Fee. 3. If it is determined that a Standard Install is not feasible, but rather a Non-Standard Install is necessary, customers choosing NOT to follow through with Non-Standard Install will be assessed a $50 fee and charged to the customer’s credit card. 4. OTHER FEES MAY APPLY BASED ON ACTUAL SITE SURVEY (crawl space/attic access, additional wall penetration, additional line run's, etc.) 5. Cancellation of a system installation or service work is subject to a $50 SERVICE CHARGE unless THE SATELLITE GUY, INC. has received prior notification at least 24 hours prior to the scheduled appointment 6. THE SATELLITE GUY, INC. is NOT responsible for loss of programming due to malfunctions of satellites. 7. There will be a 20% restocking charge for returned equipment within 30 days of purchase and must be returned in the same condition as originally purchased with the original packaging. No returns will be accepted after 30 days. 8. In the event that equipment must be returned to THE SATELLITE GUY, INC., please call for a Returned Merchandise Authorization number. THE SATELLITE GUY, INC. will not accept returns without an accompanying  RMA number. 9. In a multi-receiver installation, only one receiver is billed the full programming rate. Additional receivers are charged $4.99 each per month to activate for the same programming that is on the primary receiver. All multi-receiver systems must be connected to the same phone number. In the event Customer fails to meet any of these terms and conditions, THE SATELLITE GUY, INC. will impose a $250 termination fee for the 1st receiver & $60 each additional receiver. Customer will also reimburse THE SATELLITE GUY, INC. for the cost of all equipment. These fees will be charged to customer’s credit card on file, if any, or billed to customer and due within 10 days of invoice date.

In the event Customer fails to meet any of these terms and conditions listed above, THE SATELLITE GUY, INC. will impose a $300 termination fee for the Primary receiver & $50 for each additional receiver.

In Addition, the customer may also reimburse THE SATELLITE GUY, INC. for the cost of all equipment (See below for specific equipment charges). These fees, which are in addition to the Dish termination fees set forth above, or any other equipment/service fees due and not paid for at time of service will be charged to customer’s credit card on file with THE SATELLITE GUY, INC. or Dish Network, if any, or billed to customer and due within 10 days of invoice date. THE SATELLITE GUY, INC. reserves the right to modify the fees, and/or terms and conditions of sale without prior notice.  Customer agrees to pay all fees associated with collection of debt that could be incurred by customer due to not meeting the contract requirements .

The costs for equipment is as follows for reimbursement to The Satellite Guy, Inc.  You will be charged for each item you originally signed up for.

Receivers: Model 311 = $119, Model 510 = $310, Model 522 = $369, Model 811 = $419, Model 322 = $179, Model 721 = $569, Model 921 = $799
Dish and other equipment: 24" dish = $79, 30" dish - $89, DP34 Switch = $79, DP LNB = $49, Legacy LNB = $39, 4-way Multi Switch = $49.
